The Health Center at Bloomingdale, a five-star skilled nursing facility, offers short and long-term nursing and rehabilitation care in a secure and caring environment. When a resident is admitted to our home, our skilled team of trained professionals meets with the resident and family member, providing a unique and highly personalized plan of care. Our residents are continually assessed for changes to optimize their function and quality of life. Our plan of care always reflects the residents' individual and personal choices.
Responsibilities for LPN - Licensed Practical Nurse:
Assist with resident assessment and implementation of care needs and will participate in resident and family care conference
Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public
Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented
Provide support and assistance to medication management systems
Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored
Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management
Requirements for LPN - Licensed Practical Nurse:
Completion of an accredited school of practical nursing
Current licensure as a Practical Nurse ( LPN ) in the State of New Jersey

Experience: 1
year preferred but not required
